Beginning January 01, 2019

Ohio Department of Taxation Employer Withholding Tax Filing Guidelines

Notice – As passed by a majority affirmative vote of all residents during the May 8, 2018 election, an income tax of 1.25% on the earned income of individuals residing in the school district shall be imposed by the Madison-Plains Local School District, for 5 years, beginning January 1, 2019, for the purpose of current expenses.

Employers (regardless of location) are required to withhold and remit this tax for Madison-Plains Local School District residents only.  It is recommended that you confirm this new withholding is occurring with your employer.

You can confirm your residence within Madison-Plains Local School District after 1/1/2019 via the online Ohio Department of Taxation resource “The Finder”.  Simply enter your address after 1/1/2019 and click “Lookup”.  

The Madison-Plains Local School District “School District Number” is 4904.
