Physicals by Dr. Smith
Dr. Eric Smith with be doing physicals for sports and band here at Madison-Plains High School in the gym on the following dates:   May 22,  from 3pm-6pm and July 24, from 2-5pm.  
 
The costs for a physical is $10, checks can be made payable to Dr. Smith.  Physicals forms can be found on the athletic page under forms or can be picked up in the jr. high/high school office.  Forms must be completely filled out and signed by a parent if the parent isn't going to be present at the physical exam.

Summer lunch program starts soon
Did you know there are free activities and meals available in our community for kids and teens this summer under the age of 18? The Summer Lunch Program is offered by the Food and Nutrition Service  division of the U.S. Department of Agriculture and there is no cost to enroll. Find a site near you by calling 1-866-348-6479 or visiting www.fns.usda.gov/summerfoodrocks. 
Safe School Helpline
If you see something, say something! Madison-Plains is now using The Safe School Helpline (through a donation to the district), a convenient, confidential way for anyone to report information about situations that might affect safety at our school. Call 800-4-1-VOICE, ext. 359 or text TIPS to 66746 any time of the day or night to report concerns.

OHSAA Physicals, Emergency Medical Forms and Insurance Forms
All student-athletes must have completed the OHSAA Physical Exam Form. Click here to download the OHSAA Physical Exam form. In addition to the physical form, every student-athlete must have an Emergency Medical Form and Insurance Form on file. All forms need to be filed in the Athletic Office prior to the sports mandatory practices. A copy will be kept with the athletic trainer and coaches.
 
Hall of Fame nominations
The primary goal of MPHS Hall of Fame is to recognize those indivduals/teams that have made a significant contribution to the athletic program during their time at MPHS. If you would like to nominate someone for the Hall of Fame, print off the criteria/nomination form, fill it out and turn it in to the Athletic Department. The criteria/nomination forms can be found under athletic forms link.
