TRANSCRIPT INFORMATION

To Request a Transcript or Education Verification
follow the instructions below.
 
How to Request a Transcript
Transcripts can be requested either in person or by mail. You must provide a photo ID along with a $2.00 fee for each transcript request. Method of payment includes money order, check or cash. No request will be processed without fee enclosed. Once a request is received, it will be processed within 24-48 hours. Please note that we do not accept any requests for transcripts over the phone. In compliance with the Family Educational Rights and Privacy Act of 1974 (FERPA), Requests for individuals who have graduated and are 18 years or older must come directly from that individual. Instructions for Walk-In Transcript Requests
1. Requests are made at Madison-Plains High School in the High School Office.
2. Fill out the "Transcript Request Form".
3. Make sure you provide your full name when you attended High School (including Maiden Name) 
4.. Bring a photo ID and $2.00 fee.
5. We can mail, fax or email the transcript to wherever you are needing it to go to or you can take it with you right then.
 
Instructions for Mail in Transcript Requests
1. Print off and fill out the "Transcript Request Form" located online.
2. Make sure you provide your full name when you attended high school (Including Maiden Name). 
3. Provide a copy of your photo ID.
4. There is a $2.00 for each transcript request. We accept check, money order, or cash. Made payable to: Madison-Plains high school. 
4. Mail completed form along with photo ID and $2.00 fee to: 
    Madison Plains High School, Attn: Records, 800 Linson Rd. London, OH 43140
5. Once request is received, we will mail, fax or email your transcript to wherever you need it to go.
 
Instructions for Education Verifications of former Graduates
1. Education Verification requests must be on company letterhead along with the graduate/non-graduate signed release. (electronic signature accepted).
2. We will not provide information over the phone without the request and release.  
3. You must provide students full name (maiden name, if applicable) and date of birth. 
4. You can mail, fax, or email your request provided below. 
5. Once received, we will mail, fax or email your request within 24-48 hours.
